Booking Your Supported Holiday
As our holidays get booked up quickly, we would recommend you secure a place as soon as possible to avoid disappointment. Please be aware we operate on a first come, first served basis.

Please provide full, complete and accurate information, continuing on a separate sheet if necessary. We need to know exactly what support you require with regard to your personal care, mobility and medication. Could we kindly request that you do not assume the holiday staff know the holidaymaker and therefore omit relevant and appropriate information.
The completed form should be returned to: Morley Care Services Limited, The Wesley Centre, Blyth Road, Maltby, Rotherham, S66 8JD, with your 25% deposit. Cheques should be made payable to Morley Care Services Limited. NB. If the travel date is within 90 days, the full cost of the holiday is payable.
We will assess the information you provide and decide whether we can meet your support needs. We may need to contact you for further information. We will write to confirm your place on the holiday with further details. If we believe you require additional support, we will contact you to discuss your options.
Please note that if you omit information on your booking form which has an impact on your holiday and the group as a whole, you may be asked to leave your holiday early.

We are able to offer 1 to 1 support on our holidays to those who require additional support with their personal care, mobility and medication. There will be an additional cost for this service but it means you can enjoy a group holiday with tailored support. Please contact us for more details.
